I hereby represent, warrant and confirm that I have independently and of my own volition decided to take up the Non-Resident India (NRI) product/facility and services available with ICICI Bank Limited, India ('ICICI India') and/or make enquiries about it. In this connection, I have of my own accord personally requested ICICI India, solely for my benefit to collect, use and disclose my personal data to ICICI Bank Limited, Singapore branch ('Branch') for any services availed and/ or to be availed by me through ICICI India and I hereby authorize the Branch, purely as a communication conduit, to facilitate the following acts in connection with my application for or interest in the NRI product/facility and service, whether online or through other channels of communication of the Branch, including contacting me on my registered email and telephone (as available with ICICI India and shared with the Branch per my consent above):

Information on products/facilities

1. I further acknowledge and confirm that no solicitation or offer, or attempts to solicit or offer, has been made by the Branch or by any other branches or subsidiaries of ICICI India in Singapore in relation to the NRI product/ facility and service or any other product or service of ICICI India.
2. I further acknowledge and confirm that none of ICICI India or the Branch have provided any information pertaining to the NRI product / facility/ service without my specifically requesting for the same. I confirm that no advisory services have been provided by any of the branches or subsidiaries of ICICI India (including the Branch) with regard to the above identified product / facility/ service
3. I hereby agree to fully indemnify ICICI India and the Branch against any and all losses, damages, costs, expenses, liabilities, actions and proceedings whatsoever arising from or in connection with ICICI India or its Branch agreeing to accede to my request for assistance herein and/or carrying out such actions as requested by me.
4. I hereby acknowledge, agree and confirm that the Branch is not responsible or liable for the NRI product / facility and service or in determining the authenticity or validity of the documents (if any) provided by me with regard to the NRI product / facility and service, and its agreeing to render such assistance as may be requested by me (which it shall be entitled to refuse) shall not be taken in any way to be a recommendation or endorsement of my choice of such product/ facility and service.

PERSONAL DATA CONSENT

I hereby authorise, agree, consent and allow ICICI India (as defined above), and/or any of its affiliates, branches or subsidiaries (collectively referred to as 'ICICI Entities'), and their respective employees, agents, and third party service providers to collect, use, disclose, process my personal data in relation to my use or availing of or access to any of the accounts held (or proposed to be held) by me and for related services/ facilities availed by me from ICICI India. Further, ICICI Entities may collect, use and disclose my personal data to the Branch (as defined above) for any services availed and/ or to be availed by me through ICICI India and/ or the Branch, whether now or in future. I agree and confirm that my consent above:

1. is in addition to any other consent which I may have provided to the ICICI Entities/ Branch in respect of the collection, use and/or disclosure of my personal data; and
2. shall be without prejudice to and does not limit or derogate from ICICI Entities' (including the Branch) rights to collect, use and/or disclose my personal data pursuant to the applicable terms and conditions governing my relationship with the ICICI Entities and under applicable laws.
